A birth certificate is an original legal document recording a birth, and is issued as soon as possible after the birth. The birth certificate form is completed by the parents, a doctor or a midwife who witnessed the birth. The completed certificate is then delivered to Clerk's office, where the Registrar (City Clerk) or a Deputy Registrar (so appointed by the City Council) signs, dates, and files the certificate. The official birth certificate is then mailed to the parents.
